How Long Do Dental Implants Last? Data From Studies

It is the question almost every patient asks before they commit to treatment, and it deserves an honest answer rather than a marketing number. A dental implant is a titanium root placed in the jaw, and the crown or bridge attached to it. Both parts age, but they age differently, and the research on how long they last is now genuinely long term. Some of the studies below followed patients for two decades or more. Here is what those studies actually found.

A dental implant is a titanium root placed in the jaw, and the crown or bridge attached to it. Both parts age, but they age differently, and the research on how long they last is now genuinely long term. Some of the studies below followed patients for two decades or more. Here is what those studies actually found.

What “lasting” means in the research

Two words appear constantly in the literature, and they are not the same thing. Survival means the implant is still in place and functioning. Success is stricter: the implant survives and also meets defined criteria for healthy bone levels, no pain, no infection, and stable soft tissue. Survival figures are always higher than success figures, so when a clinic quotes a single glossy percentage, it is worth knowing which one they mean. Most of the numbers below are survival rates, because those are what the largest reviews report most consistently.

The ten-year picture

The most cited synthesis of medium to long term data pooled 18 studies of contemporary implants. It found a ten-year survival rate of 96.4% at the implant level, with a 95% confidence interval of 95.2% to 97.5%.1 Put plainly, of every 100 modern implants placed, roughly 96 were still doing their job a decade later. That figure is one reason implants have become the reference standard for replacing missing teeth rather than an experimental option.

A word of caution about how such numbers are produced. The same review ran a sensitivity analysis that assumed the implants lost to follow-up had failed, and the estimate dropped to around 93%. Real clinical performance sits somewhere between the optimistic and the conservative reading, which is closer to how we prefer to talk about it with patients.

Fifteen, twenty years, and further

Longer follow-up naturally shows more attrition, and this is where honest reporting matters most. A cohort study of 10,871 implants placed in 4,247 patients, followed for up to 22 years, reported implant-level survival of about 94% at 15 years.2 A separate 2024 meta-analysis focused specifically on the 20-year mark. Across the prospective studies it analysed, survival was around 92%, and a pooled estimate that accounted for missing data landed lower, closer to the high 70s to high 80s depending on the model used.3

The spread in those twenty-year figures is not a flaw in the implants. It reflects how hard it is to keep patients in a study for two decades, and how much the answer depends on the patient population. The reasonable takeaway is this: a well-placed, well-maintained implant has an excellent chance of serving you for the rest of your life, but no clinician can honestly promise 100% permanence.

What actually shortens implant life

Failures cluster into two types. Early failures happen in the first months, before the bone fully integrates. Late failures happen years later and are usually biological (peri-implant infection and bone loss) or mechanical (fracture of a component or the restoration). In the 22-year cohort, several factors raised the risk of failure measurably: smoking above ten pack-years increased the hazard by roughly 80%, and diabetes roughly doubled it.2

  • Smoking is the single most consistent modifiable risk across the literature.
  • Uncontrolled diabetes and other systemic conditions that impair healing.
  • Peri-implantitis, the inflammatory bone loss driven by plaque around the implant, which is why cleaning matters as much here as with natural teeth.
  • Heavy grinding (bruxism) and excessive bite forces, which stress the mechanical parts.

Complex cases and the atrophic jaw

Not everyone has enough bone for a standard implant, and the longevity question changes shape for these patients. When the upper jaw has severe bone loss, zygomatic implants (anchored in the cheekbone) and other advanced techniques can restore a fixed set of teeth where conventional implants would not hold. One 20-year retrospective study of patients treated with zygomatic implants alongside conventional anterior implants reported conventional implant survival of about 90% at ten years, declining across the second decade in this more demanding group.4 The message is not that these solutions are fragile, but that outcomes in atrophic jaws need to be judged against the alternative, which is often no fixed teeth at all.

How care changes the numbers

The durability figures above are not fixed by fate. They shift with what happens after placement. Studies repeatedly show that patients enrolled in a regular maintenance programme develop far less peri-implant disease than those who do not attend. Daily cleaning around the implant, professional maintenance a few times a year, stopping smoking, and managing conditions like diabetes are the levers that move a patient from the lower end of these ranges toward the upper end.

An implant is closer to a joint replacement than to a filling: excellent, long lasting, and dependent on the care it receives. How long do dental implants last?

The systematic review and meta-analysis by Howe and colleagues shows ten-year survival at implant level of around 96.4 percent, or around 93.2 percent when patients lost to follow-up are counted in more methodologically strictly, with somewhat weaker results in people over 65. A more recent meta-analysis by Kupka and colleagues follows implants for up to 20 years and shows that roughly four out of five implants remain in function even after two decades. Longevity is a process, not just the moment of the procedure.

Does the crown on an implant last as long as the implant?

No, and it is useful to separate the two parts. The titanium part in the bone is the most durable element of the system, and it is what the survival figures across decades refer to. The visible part, the crown, bridge or denture on the implant, is more exposed to wear, chewing load and time. It is not unusual for a titanium implant to remain flawless while the crown on it is renewed once during a lifetime, which is ordinary maintenance rather than a failure of the placement.

Can I smoke if I have dental implants?

Smoking measurably increases the risk. The meta-analysis published by Reis and colleagues shows that smokers have roughly two to almost three times the risk of peri-implantitis compared with non-smokers, and peri-implantitis is the most common biological reason for later implant loss. Stopping smoking measurably reduces that risk.

How do you extend the life of a dental implant?

Daily, thorough cleaning, including interdental brushes or a water flosser around the implant, makes the biggest difference. Stopping smoking measurably reduces the risk, and regular professional check-ups and hygiene make it possible to catch mild inflammation before it reaches the bone. For patients with a history of periodontitis we recommend somewhat more frequent check-ups, because for them an earlier response also means longer service.

Do dental implants come with a guarantee?
